This is critical for individuals with severe allergic reactions or sensitivities, ensuring they can safely take their prescribed medications.

3. Discontinued or Unavailable Drugs

When a commercially available drug is discontinued or on backorder, compounding pharmacies can recreate the medication based on the original formulation. This ensures continuity of care and access to essential treatments that might otherwise be unavailable.

4. Hormone Replacement Therapy (HRT)

Compounding plays a significant role in bioidentical hormone replacement therapy (BHRT), where formulations are tailored to mimic the hormones naturally produced by the human body. This is often used in managing menopause, andropause, or thyroid conditions, and allows for personalized adjustments based on hormone levels and patient response.

Despite their clear benefits, compounding pharmacies operate in a complex regulatory and clinical environment. They face challenges related to oversight, standardization, and public perception.

In the U.S., compounding pharmacies are primarily regulated by state pharmacy boards, with additional oversight from the Food and Drug Administration (FDA) in certain contexts. The 2013 Drug Quality and Security Act (DQSA) established a category of “outsourcing facilities” that must comply with federal manufacturing standards. However, traditional compounding pharmacies still fall under a different set of less stringent regulations.

This dual system can lead to variations in quality and safety standards, prompting concerns about sterility, potency, and consistency. High-profile incidents—such as the 2012 fungal meningitis outbreak linked to contaminated compounded drugs—have underscored the need for robust regulatory frameworks and enforcement.

Lack of Clinical Trials

Unlike FDA-approved drugs, compounded medications do not go through formal clinical trials. While they are based on established medical principles and formulations, the absence of rigorous, standardized efficacy and safety data can be a barrier to broader acceptance among healthcare providers.

Public and Provider Perception

There is a perception among some healthcare professionals that compounded medications are less reliable than commercial drugs. While this is not necessarily true—especially in well-regulated pharmacies—it highlights the need for greater transparency, standardization, and education.

The Future of Compounding in Personalized Medicine
